Monthly Accountability
Check-In
Check-In
This is not a reflection exercise. It's a reckoning. You made commitments. Let's look honestly at where you are — not to judge you, but because your goals are worth the truth.

Getting started
Let's begin.
Every month you complete this, you're choosing your goals over your comfort zone. That decision alone separates the clients who get results from those who don't.
Section 1 of 5
Wheel of Life
Rate each area exactly as it is right now — 1 is rock bottom, 10 is genuinely thriving. This is your compass. The gaps between scores are where your work lives.
Be specific. "Better" is not an answer. What would genuinely change? What would you see, feel, have, or do differently?
Section 2 of 5
Your Committed Goals
These are the goals you said matter. Not wishes — decisions you made out loud in a coaching session. Goals can be business strategy (publish Meta ads, close 3 clients), mindset (reframe thoughts around rejection), habits (daily movement, 8hrs sleep), or personal (improve a key relationship). All of it counts. All of it is connected.
If your answer is "I need to change them" — ask yourself honestly: is this a genuine pivot, or is your brain finding a comfortable exit?
Section 3 of 5
Session Homework —
Did You Do The Work?
Did You Do The Work?
In your last session, you identified goals, worked through blocks, and committed to specific actions. This section is where we find out if those commitments stayed commitments — or became good intentions.
This might be a business task, a mindset practice, a habit to start or stop, or a conversation to have. Write what you committed to.
If you completed it: what made it possible? If you didn't: be honest about what stopped you. Not to criticize yourself — but because your pattern is your most important data point.
The thing that was keeping you stuck that your session helped you identify.
Section 4 of 5
The Thought Model —
Are You Using It?
Are You Using It?
The Thought Model is your tool for catching the thinking that creates the results you don't want. It only works if you use it. First, map your current (unintentional) model. Then use the Reverse Model to deliberately choose a new thought and follow it forward to a better result.
Left side: your unintentional model — what your brain actually did. Right side: your intentional model — what you choose to think instead, and where that leads. The reframe only works if the new thought is believable to you.
⚠️
Unintentional Model
What your brain did automatically
C — Circumstance
Just the facts. No opinion.
T — Thought
The story you told yourself.
F — Feeling
One word.
A — Action
What you did (or didn't do).
R — Result
What that created.

✦
Intentional Model
What you choose to think instead
C — Circumstance
Same fact. Unchanged.
T — New Thought
Must be believable to you.
F — New Feeling
One word.
A — New Action
What you'd do from this feeling.
R — New Result
What that would create.
Section 5 of 5
Patterns, Momentum
& What's Next
& What's Next
Your brain is brilliant at keeping you comfortable and calling it "realistic." This section is where you look your pattern in the eye — with compassion, and with honesty.
Be specific. What did you actually do that moved you forward?
This is not about being hard on yourself. It's about being honest with yourself. The gap between your intentions and your actions is exactly where your coaching work lives.
Check all that apply — no judgment, just data.
Not "try." Not "hope to." Will. And name the thing most likely to get in the way so you can decide now how you'll handle it.
